LETTER OF DISCLOSURE AND CONSENT
In complying with the FAIS legislation, I would like to bring the following information to your attention:
My name is Johannes F. van Schalkwyk, employed by the Beetro Group, FSP 49552, an authorised financial services provider, which accepts responsibility for my activities; and is licensed to render financial services. I have been providing financial advice and intermediary services since 2002 in the following areas of financial planning: Death and Disability Planning; Retirement Planning; Investment Planning; Estate Planning.
I am authorised to provide advice and intermediary services in the following FAIS categories:

The Beetro Group has written authority to market the relevant products of most of the reputable product suppliers, and I am accredited to market their products.
The Beetro Group did not receive more than 30% of total annual remuneration from any of the product suppliers that we place business with.
Neither I nor the FSP holds more than 10% of the shares issued by any product supplier.
I am being paid commission by the product provider for intermediary services provided, at acceptance of the financial product by the product provider and the client, only when the policy document is active. At cancellation of the product by the client or the product provider, there may be a claw back of commission by the product provider. In such a case, you, as the client, may be held liable for the cost of the intermediary services provided.
I comply with the fit and proper requirements as defined in the FAIS Act and hold professional indemnity as required. Compliance with the FAIS Act is monitored by Providus Compliance Consulting (Pty) Ltd, an approved external compliance practice. Their address is 29 Milton Road, Goodwood, 7460. This support helps me to provide you with a more professional service. The compliance service enables my practice to be compliant with FAIS legislative requirements. Through the practice management support, I run a professional business and therefore able to provide you with an improved service and enhanced support.
We have a structured complaint system. Please note: We always strive to render services and provide advice to our clients that are in their best interest, and to avoid any possible conflict of interest that may arise. We keep an updated Conflict of interest policy & Disclosure register. This informs you, our client, of all financial and ownership interests that I/ we may become entitled to and lists the business relationships that we have with the product suppliers. This document ensures transparency in my/our dealings with our customers and is available for inspection.
Privacy Notice: Our clients can rest assured that we will respect and protect their rights to privacy and confidentiality of their personal information collected, processed, and stored by us. Due to the nature of our business and in the process of the fulfilment of our contractual obligations to our clients we will have to collect and process your and/or your family’s personal information and special personal information. We will only require and process your personal Information and special personal information for the purposes of rendering our services to you in a competent and professional manner, meaning it will only be processed for the purposes collected and we will ensure that the information obtained is adequate, relevant, and not excessive. In the process of the fulfilment of our contractual obligations to our clients we will have to share some, or all of the personal information and special personal information obtained from a client with product providers and/or operators in order for us to be able to provide you with suitable advice and/or products and to keep records as required of us in legislation applicable to our industry. We will not share any personal information and special personal information with any product provider and/or operator unless we are satisfied that they have sufficient safeguards in place to convince us that they will respect and protect our clients’ right to privacy with the same commitment as us.

I hereby consent to the collection, processing and storing of my and my family’s personal and special information, and I hereby give consent to the collection, processing and storing of my minor children (if any) personal information.
I herewith give consent to the The Beetro Group duly appointed personnel to share as much of my/our personal information and special personal information with the necessary product providers and/or operators in order for The Beetro Group to provide me/us with suitable advice and/or products as required, and to keep records as required of The Beetro Group is in terms of applicable legislation.
